Licensed Pharmacist

• Review, transcribe, and accurately enter medication orders.
• Compound and dispense medications safely and accurately.
• Monitor drug therapy for interactions, allergies, contraindications, and appropriate dosing.
• Support formulary management and therapeutic substitutions.
• Provide medication information to healthcare staff and patients.
• Assist with clinical services including pharmacokinetics, anticoagulation, pain management, renal dosing, nutrition support, and IV-to-PO therapy.
• Document clinical interventions and medication-related activities.
• Complete medication error and adverse drug reaction reports.
• Supervise pharmacy operations and support the pharmacy team.

• Minimum 1 year of licensed pharmacist experience.
• Acute inpatient hospital experience preferred but not required.
• PGY-1 residency may be considered in place of inpatient experience.
• B.S. or PharmD from an ACPE-accredited School of Pharmacy required.
