LONG-TERM CARE PATIENT CARE POSITION
We are seeking an experienced healthcare professional to provide supportive, compassionate care to residents in a long-term care setting.
RESPONSIBILITIES:
- Provide supportive services to nurses and other clinical staff as assigned.
- Assist residents with activities of daily living according to care plans and established procedures.
- Assist with lifting, transferring, repositioning, and transporting residents using beds, wheelchairs, lifts, chairs, and other equipment.
- Assist with dining services, including meal setup, tray delivery, feeding assistance, cleanup, and documenting meal intake.
- Deliver nutritional supplements and assist residents as needed to ensure proper intake.
- Assist with resident weights and report significant changes to nursing staff.
- Monitor residents for skin changes, pressure areas, breakdowns, or skin tears and report concerns promptly.
- Assist nursing staff with toileting programs.
- Complete daily flow sheets and required resident documentation.
- Report changes in resident condition to the nurse and supervisor.
- Communicate any issues that prevent assigned duties from being completed.
REQUIREMENTS:
- Minimum 1 year of Long-Term Care (LTC) experience – REQUIRED.
- High School Diploma or equivalent – REQUIRED.
- Previous travel healthcare experience – REQUIRED.
- Current BLS through AHA – REQUIRED.
- Must have a stable work history, with previous assignments completed beyond 13 weeks.
- No active disciplinary action against required certification.
- Active Michigan license/certification at time of submission – REQUIRED.
SUBMISSION REQUIREMENTS:
- Discipline-specific skills checklist completed within the past year.
- Two supervisory references from the last 2 years of employment.
- Active license/certifications at time of submission.
- Current AHA BLS certification.
