LPN – SNF/LTC
JOB SUMMARY:
The LPN provides nursing care to residents/patients under the supervision of the Unit Manager/ADON/DON. Responsible for assessing residents, implementing and evaluating care plans, maintaining a safe environment, and supervising nursing assistants and support staff.
KEY RESPONSIBILITIES:
- Provide nursing care in accordance with established standards and the State Nurse Practice Act.
- Assess residents and implement/evaluate individualized plans of care.
- Monitor resident safety and maintain a supportive care environment.
- Supervise nursing assistants and helpers in resident care activities.
- Follow facility policies, procedures, and applicable quality standards.
- Provide excellent customer service and compassionate resident care.
- Demonstrate professionalism and teamwork throughout the assignment.
REQUIREMENTS:
- 2+ years of SNF/LTC experience – Required.
- Active Michigan LPN license – Required.
- BLS through AHA or American Red Cross – Required.
- COVID-19 vaccination – Required; medical/religious exemptions may be considered case-by-case.
- Previous travel experience – Required.
- Flexible scheduling required — blocked scheduling cannot be guaranteed.
