Core Requirements for Fast-Track Presentation
To ensure a seamless name-clear and presentation process, candidates must meet the following baseline criteria:
- Licensure & Certification: Active Michigan State License and Board Certification as an NP Hospitalist are strictly required.
- Location Logistics: Must reside in or near Michigan as a local drive-in candidate
- Background Check: A fresh Self-Query NPDB pulled within the current month must be provided at the time of presentation.
Employee Health Requirement
a. Flu vaccine for 24-25 season
b. Covid documentation - If 24-25 booster has not been received a declination is needed
c. TB - 2 step skin test within 12 months or Quantiferon within the last 6 months
d. TB Questionnaire
e. Hep B - titer with quantifiable numbers or declination
f. MMR - vaccine series or titers with quantifiable numbers
g. Varicella - vaccine series or titers with quantifiable numbers
h. TDAP - adult vaccine (within the last 10 years)
