About the Job
Temp Community Health LPN.
Provides and documents nursing care to patients in their homes, promoting health, preventing illness, supporting rehabilitation, and coordinating care with other healthcare professionals. Must work independently, exercise sound clinical judgment, and demonstrate strong critical thinking, communication, and time-management skills.
Requirements:
- Minimum 1 year of acute nursing experience; hospital-based experience preferred.
- Home health experience required.
- Certified Home Health experience and OASIS proficiency highly preferred.
- Infusion therapy experience preferred.
- Active Massachusetts LPN license in hand at submission.
- Current BLS certification through AHA or American Red Cross.
- Valid driver’s license, auto insurance, and personal vehicle required.
- Knowledge of home health reimbursement, regulatory guidelines, and community health practices.
- Ability to work independently, maintain confidential records, and accept on-call responsibilities.
- Flexibility with work hours, scheduling, and holiday coverage.
