Home Health Registered Nurse
Job Summary
Seeking a Registered Nurse to provide comprehensive home health care, coordinate patient services, develop and manage plans of care, and educate patients and caregivers while maintaining compliance with regulatory standards.
Responsibilities:
- Provide nursing care through the nursing process and maintain professional nursing standards.
- Supervise CNAs and coordinate care with other healthcare professionals.
- Complete comprehensive initial and ongoing patient assessments.
- Assess physical, psychosocial, environmental, and community needs.
- Initiate, evaluate, and modify Plans of Care as needed.
- Establish discharge plans with patients, physicians, caregivers, and other disciplines.
- Complete OASIS assessments and required documentation.
- Provide patient care in accordance with Federal, State, and agency regulations.
- Assess patient and caregiver education needs and provide appropriate teaching.
- Maintain accurate documentation according to regulatory requirements.
- Promote a clean and safe patient care environment.
- Maintain patient privacy and confidentiality.
Requirements:
- Home Health experience required.
- Active BLS or willingness to obtain required.
- Valid driver's license and auto insurance required.
- Active Illinois nursing license required.
- Strong Med/Surg experience with some Home Health experience may be considered.
- OASIS and Start-of-Care experience strongly preferred.
- COVID and Flu vaccination required; declinations will not be approved.
- Local candidates preferred based on territory coverage.
