Responsibilities
✓ Conduct initial and ongoing patient assessments.
✓ Develop and update individualized plans of care.
✓ Complete OASIS assessments and Start of Care documentation.
✓ Provide patient and caregiver education.
✓ Coordinate care with physicians, CNAs, and other healthcare professionals.
✓ Maintain accurate documentation and follow nursing standards and safety regulations.

Requirements
✓ Active Illinois RN license.
✓ Home health experience required; strong Med-Surg experience with some home health experience may be considered.
✓ Active BLS or willingness to obtain.
✓ Valid driver's license and car insurance.
✓ COVID-19 and flu vaccinations required; declinations not accepted.
✓ OASIS and Start of Care experience strongly preferred.
✓ Ability to travel within the assigned territory.
