Director of Regional Care Management (Interim)
Seeking an experienced healthcare leader to oversee care management operations across a large hospital system. This role focuses on improving efficiency, reducing readmissions, strengthening discharge planning processes, enhancing team collaboration, and driving operational performance across case management, social work, and care coordination teams.
Key Responsibilities:
- Lead and optimize care management, utilization management, and discharge planning programs.
- Partner with physicians, revenue cycle, and interdisciplinary teams to improve patient outcomes and operational efficiency.
- Analyze workflows, identify process improvements, and implement strategies to increase productivity and team engagement.
- Monitor performance metrics, regulatory compliance, and care management outcomes.
- Foster collaboration between behavioral health and medical social work teams.
- Manage high patient volumes across multiple units while ensuring quality and compliance with federal and state regulations.

- Proficient with Microsoft Office tools, including Excel and Power BI.
- Strong understanding of care management, utilization review, and healthcare operations.
Required Qualifications:
- Minimum 3 years of recent care management, case management, or social work experience.
- Minimum 3 years of progressive or interim leadership experience.
- Experience supervising 10+ FTEs, PRN staff, or contractors.
- Experience in a large healthcare system or hospital with 600+ beds.
- BSN, Licensed Independent Social Worker (LISW), or equivalent healthcare management education.
- Active Iowa RN license (or Compact/Multistate license if applicable) for nursing candidates.
- BLS certification.
- Valid driver's license.
Preferred Qualifications:
- Epic EMR experience.
- Master's degree in Nursing, Healthcare Administration, Business, or related field.
- Experience with Strata/financial management systems.
Contract Details:
- Two rounds of virtual interviews required.
- Orientation dates available in July.
- Holiday coverage may be required based on departmental needs.
