Physical Therapist (PT)

POSITION SUMMARY:
This role is responsible for delivering patient-focused physical therapy care through assessment, planning, intervention, and evaluation in accordance with physician orders, therapy standards, and clinical policies.

The Physical Therapist also provides delegation and oversight to Physical Therapist Assistants (PTAs) and Aides to ensure high-quality patient care and treatment outcomes.

CORE RESPONSIBILITIES:
  • Evaluate patient function using formal and informal testing methods to assess skeletal, neurological, respiratory, and cardiovascular status.
  • Develop individualized treatment plans based on assessment findings and patient tolerance.
  • Create and submit written treatment plans to be coordinated with the physician and included in the overall plan of care.
  • Update plans of care as patient conditions change.
  • Co-sign verbal orders received by Physical Therapist Assistants as needed.
  • Establish treatment schedules with frequency and duration specific to patient needs.
  • Coordinate care plans with patients, families, caregivers, and clinical support staff.
  • Educate patients and families regarding diagnosis, prognosis, treatment options, exercise programs, modalities, and rehabilitation expectations.

SUBMISSION REQUIREMENTS:
  • Physical Therapy experience required.
  • Pediatric experience required.
  • Acute Care experience highly preferred.
  • Active Michigan Physical Therapist License required (must be in hand at time of submission).
  • Active American Heart Association (AHA) BLS certification required.

REQUIRED DOCUMENTS:
  • Updated Work History.
  • Skills Checklist (current within 1 year and specific to PT specialty).
  • 2 Supervisory Reference Forms completed within the last 2 years.
  • Proof of active Michigan PT License (LARA and/or Nursys verification).
  • Active AHA BLS certification documentation.
