RN CARE MANAGER – HOSPICE.

SHIFT: DAYS.
Schedule: Monday–Friday.
Hours: 8:30 AM – 5:00 PM.
Mileage Reimbursement: Available.

JOB DESCRIPTION:

The RN Care Manager provides compassionate, high-quality nursing care to hospice patients and their families. This role requires strong bedside nursing skills, particularly in symptom management, crisis intervention, and family education.

KEY RESPONSIBILITIES:

Manage patient care and provide specialized nursing services to hospice patients.
Assess and manage patient symptoms and respond to care-related crises.
Educate patients and families on care plans and symptom management.
Develop and maintain positive relationships with patients and families.
Collaborate with the interdisciplinary healthcare team to ensure quality patient care.
Maintain effective written and verbal communication and demonstrate strong problem-solving and decision-making skills.
Provide care primarily in patients’ homes, with travel to long-term care facilities, hospitals, and other locations within the service area.

REQUIREMENTS:

Graduate of an accredited nursing school – Required.
Active Virginia RN license, verified through Nursys – Required.
Minimum of 2 years of experience in acute care, home health, hospice, medical-surgical, or geriatric nursing – Required.
Current Virginia driver's license – Required.
Driving record must meet applicable policy standards – Required.
BLS (AHA) certification – Required.
