NOW HIRING: HOME HEALTH REGISTERED NURSE (RN)
- Schedule & Hours
- Monday–Friday.
- Day Shift: 8:00 AM – 5:00 PM.
- 40 hours guaranteed per week.
Responsibilities:
- Perform comprehensive patient assessments and ongoing evaluations.
- Develop, implement, and update individualized plans of care.
- Complete OASIS assessments and required documentation.
- Coordinate patient care with physicians, CNAs, caregivers, and other healthcare professionals.
- Establish discharge plans and provide patient and family education.
- Ensure compliance with applicable state, federal, and agency regulations.
- Maintain patient privacy, confidentiality, and a safe care environment.
Requirements:
- Home Health experience required.
- Active BLS certification or willingness to obtain.
- Valid driver's license and car insurance.
- Active Illinois nursing license required at submission.
- COVID-19 and flu vaccinations required; declinations will not be accepted.
- Strong Med-Surg experience with some Home Health experience may be considered.
- OASIS and Start of Care experience strongly preferred.
- Local candidates preferred.
