Hospice Registered Nurse (RN) – Case Management

JOB SUMMARY:

Seeking an experienced Hospice RN with at least 1 year of current hospice case management experience. The RN will manage a caseload of approximately 15 patients and provide comprehensive hospice nursing services based on patient needs and acuity.

JOB DETAILS:

• Manage a caseload of approximately 15 patients.
• Complete 4–5 patient visits per day, depending on service area and patient acuity.
• Complete charting the same day as visits.
• Provide all levels of hospice nursing care, including:
  • Routine hospice care.
  • General Inpatient (GIP).
  • Continuous care.
  • Respite care.
• Perform admissions, Symptom/Intensive Assessment (SIA), and death visits.
• Cover a service radius of approximately 60 miles.
• Mileage reimbursed at the applicable IRS standard mileage rate.
• Clinicians may float to Oregon sites.

REQUIREMENTS:

• Minimum 1 year of current Hospice Case Management experience – REQUIRED.
• Active Washington RN or Compact license – REQUIRED.
• Active American Heart Association BLS – REQUIRED.
• Valid driver's license and reliable vehicle – REQUIRED.
• Valid auto insurance – REQUIRED.
• Clean background check in Washington and Oregon.
• Successful drug screening.
• Influenza, TB, and COVID vaccination records/documentation.
• Oregon RN license strongly encouraged.
• Oregon RN licensure application is required upon accepting the contract if not already licensed.

PROFILE/SUBMISSION REQUIREMENTS:

• Updated resume/work history; gaps of 30+ days must be explained.
• Hospice-specific Skills Checklist.
• 2 supervisory references from the last 2 years of employment.
• Active Washington or Compact RN license.
• American Heart Association BLS certification.

LICENSE & CERTIFICATION:

• Washington RN License or Compact License – Required.
• Oregon RN License – Strongly Encouraged.
• BLS – American Heart Association – Required.
