LPN – SKILLED NURSING / LONG-TERM CARE
We are seeking an experienced Licensed Practical Nurse (LPN) for a Skilled Nursing Facility/Long-Term Care assignment. The LPN will provide direct nursing care, assess and implement care plans, ensure resident safety, and supervise nursing assistants and support staff.
Responsibilities:
- Provide quality nursing care to residents in a SNF/LTC setting.
- Assess, implement, and evaluate individualized plans of care.
- Administer medications and treatments according to facility policies and the State Nurse Practice Act.
- Monitor resident conditions and communicate changes to the appropriate clinical team.
- Supervise and support CNAs and other nursing support staff.
- Maintain accurate documentation and promote a safe care environment.
- Deliver exceptional customer service and compassionate resident care.
Requirements:
- Active Michigan LPN license required at the time of submission.
- Minimum of 2 years of recent SNF/LTC experience required.
- Previous travel experience required.
- Current BLS certification through the American Heart Association or American Red Cross.
- COVID vaccination required; medical and religious exemptions may be considered on a case-by-case basis.
- Agency must have an active Michigan iCHAT account and authorization to schedule fingerprinting.
- Must be flexible with scheduling; blocked scheduling cannot be guaranteed.
