LPN – SKILLED NURSING / LONG-TERM CARE

We are seeking an experienced Licensed Practical Nurse (LPN) for a Skilled Nursing Facility/Long-Term Care assignment. The LPN will provide direct nursing care, assess and implement care plans, ensure resident safety, and supervise nursing assistants and support staff.

Responsibilities:

Provide quality nursing care to residents in a SNF/LTC setting.
Assess, implement, and evaluate individualized plans of care.
Administer medications and treatments according to facility policies and the State Nurse Practice Act.
Monitor resident conditions and communicate changes to the appropriate clinical team.
Supervise and support CNAs and other nursing support staff.
Maintain accurate documentation and promote a safe care environment.
Deliver exceptional customer service and compassionate resident care.

Requirements:

Active Michigan LPN license required at the time of submission.
Minimum of 2 years of recent SNF/LTC experience required.
Previous travel experience required.
Current BLS certification through the American Heart Association or American Red Cross.
COVID vaccination required; medical and religious exemptions may be considered on a case-by-case basis.
Agency must have an active Michigan iCHAT account and authorization to schedule fingerprinting.
Must be flexible with scheduling; blocked scheduling cannot be guaranteed.

Skills checklist completed within the past year.
BLS certification.
Michigan license verification.
References preferred.
